Interlibrary Book Request Form
Request books not available in the LMU Library or LINK+.

Interlibrary Article Request Form
Request articles not available in the LMU Library. (Faculty, staff, and EdD students only. Undergraduate and graduate students must use paper form and pre-pay interlibrary request fees.)

Research Assistants Borrowing Form (PDF)
Use this form to allow a Research Assistant to borrow materials for a faculty member or department. This form must be printed and submitted to the library Circulation Desk.

Request Purchase of Books
LMU faculty may use this form to order books for the library. Students and others may make a suggestion for the library to purchase materials using this form .

Suggest Purchase of Media Materials
Order videos, audiotapes, and CDs and DVDs online.

Library Instruction Request Form
Faculty may request that a librarian provide a class instruction on using library materials or services.

Request Media Materials in Advance
Need media items at a specific time? Book the materials using this form.

Rush Processing
Use this form to expedite processing of items in LINUS marked "1 copy being PROCESSED for LIBRARY." Items will be processed and available within 2 working days.

Reserve Services Form
Use this e-form to place items on Reserve.
