Library Barcode Request Form

This form is intended for LMU students, faculty, or staff who do not have a OneCard issued in their name. Circulation staff will generate a library barcode number for you so that you can create an account in ILLiad.

Already have a OneCard? Check the back of your OneCard to find the "Library No.", which is also your library barcode number. See a picture here.

Do you have a library account? Check by signing in with your MyLMU information here. If you do not have an account, please complete the Patron Registration Form instead. LMU students usually automatically have accounts, unless you registered late, while faculty and staff may need to register for a library account.
