Library Barcode Request Form

This form is intended for LMU students, faculty, or staff who do not have a OneCard issued in their name. Circulation staff will generate a library barcode number for you so that you can create an account in ILLiad.

Already have a OneCard? Check the back of your OneCard to find the "Library No.", which is also your library barcode number.

Do you have a library account? Check by signing into your account with your MyLMU. If you do not have an account, please complete the Patron Registration Form. LMU students usually have accounts automatically, unless they registered late. LMU faculty and staff may need to register for a library account depending on their hiring date.
